When Back Pain Isn't Really Back Pain
After a car accident, back pain is one of the most common complaints. But there is a specific pattern of back pain that is not really back pain at all. It begins in the lower back, and it travels. Down the buttock. Down the back of the thigh. Sometimes past the knee. Sometimes all the way into the foot.
This pattern has a name. Most people call it sciatica.
Sciatica is not a diagnosis in itself. It is a description of what the pain does. And when it appears after a car accident, it points to something specific happening in the spine that is often best identified early and treated before the underlying condition has time to change how the nervous system responds.
What Sciatica Actually Is
Sciatica describes pain along the path of the sciatic nerve, the largest nerve in the human body.
According to StatPearls, a clinical reference published by the National Library of Medicine through NCBI Bookshelf, sciatica represents a debilitating condition characterized by pain or paresthesias within the sciatic nerve distribution or an associated lumbosacral nerve root. A prevalent misconception often mislabels any low back pain or radicular leg pain as sciatica. Sciatica entails pain directly resulting from sciatic nerve or root pathology. Comprising nerve roots from L4 to S3, the sciatic nerve, with a diameter of up to 2 cm, stands as the body’s largest nerve.
According to Mayo Clinic Health System, despite the name, sciatica is not typically caused by a problem with the sciatic nerve. In most cases, it is caused by compression of one of the nerve roots that make up the sciatic nerve, usually the last lumbar nerve root (L5) or the first sacral nerve root (S1) as they exit the spine. The term “pinched nerve” is commonly used when describing the condition. The medical term for the condition is radiculopathy.
According to Cleveland Clinic, sciatica is a type of radiculopathy that causes pain along the sciatic nerve in the lower back and down the legs.
The distinction matters clinically. The pain is felt in the leg, but the problem is almost always in the spine.
What Causes Sciatica After a Car Accident
The most common cause of sciatica after a car accident is nerve root compression, and the most common cause of that compression is a herniated disc.
According to StatPearls, the most common origin of lumbar radiculopathy is nerve root compression. It commonly results from either disc herniation or spondylosis. A disc herniation can be either due to an acute injury or secondary to chronic degeneration of the spine. Disc herniation activates the pain fibers of surrounding tissues such as ligaments, vessels, and dura mater.
According to Mayo Clinic Health System, the most common cause of radiculopathy is a herniated lumbar disc. When disc material protrudes from the disc space into the spinal canal, it can compress a nerve root. Radiculopathy can be caused by an accident, injury, or lifting a large amount of weight improperly.
According to the Merck Manual, common causes include intervertebral disc herniation, osteophytes, and narrowing of the spinal canal (spinal stenosis). Symptoms include pain radiating from the buttocks down the leg.
For accident patients specifically, the mechanism is straightforward. The forces of a collision can cause a disc to herniate acutely. That herniation, in turn, can compress a nerve root as it exits the spine. The compression sends pain, numbness, tingling, or weakness along the entire path served by that nerve, which can extend far from the site of the actual injury.
What Sciatica Feels Like
The classic symptoms of sciatica are recognizable once described.
The pain usually starts in the lower back or buttock and radiates down one leg, following the path of the affected nerve. According to the Merck Manual, in patients with lumbosacral radiculopathy, pain radiates along the course of the sciatic nerve (symptoms related to nerve roots L4, L5, and S1), most often down the buttocks and posterior aspect of the leg.
The character of the pain often differs from ordinary muscle pain. Patients frequently describe it as sharp, electric, burning, or shooting. Some describe it as feeling like the leg is on fire, or as a pain that seems to run along a specific line rather than filling a whole region.
The pain has specific triggers. According to StatPearls, pain associated with sciatica is exacerbated by lumbar spine flexion, twisting, bending, or coughing. Sitting is often worse than standing, because sitting increases pressure on the affected nerve root.
Additional symptoms may accompany the pain. According to StatPearls, radicular symptoms may include loss of sensation and motor function depending on the severity of nerve compression. According to research published in ClinicalTrials.gov, the involvement of nerve roots such as L4, L5, and S1 can particularly affect the muscles in the lower extremities.

Why These Injuries Are So Often Missed Initially
Several factors contribute to the underdiagnosis of shoulder injuries after car accidents.
X-rays do not show soft tissue. According to StatPearls, X-rays are usually normal in rotator cuff injuries and only help diagnose large rotator cuff tears when imaging shows humeral migration over the glenoid. A patient leaving the emergency room with a clean shoulder X-ray has not necessarily been cleared of soft-tissue damage.
Symptoms can be subtle initially. Pain may not be severe at the scene of the accident. Weakness may not be apparent until the patient tries to use the arm in ways that demand the affected structures.
Attention is focused elsewhere. Emergency evaluation appropriately prioritizes life threatening injuries. A patient with a neck injury, chest trauma, or a head injury will receive evaluation for those concerns first, and shoulder symptoms may not be elicited in detail until later.
Shoulder pain is attributed to other causes. As the PubMed Central whiplash study documented, shoulder pain is frequently attributed to radiating cervical pain, missing the underlying shoulder pathology.
How Sciatica Is Diagnosed
Diagnosis begins with clinical examination, not imaging.
According to StatPearls (on lumbar disc herniation), a thorough history and physical examination are essential. The history must include questions about the quality of the pain, the impact on the patient’s activity, and the mechanism of injury.
The straight leg raise test is a common examination maneuver. According to StatPearls, it is conducted with the patient lying flat while keeping the symptomatic leg straight. The examiner slowly elevates the leg. The test is positive when it reproduces the patient’s pain and tingling at an angle lower than 45 degrees with radiation below the knee.
According to StatPearls (on radicular back pain), the physical examination assesses various myotomes and dermatomes. The independent areas of lumbosacral radiculopathy include the anterior thigh (L2 and L3), the medial calf (L4), the dorsum of the foot (L5), and the sole (S1). By identifying where sensation is altered and which muscles are weak, examiners can often identify which specific nerve root is affected.
When imaging is needed, MRI is typically the study of choice. According to the Merck Manual, diagnosis sometimes involves MRI or CT. Electromyography and nerve conduction studies can identify the affected level.
Imaging is generally not the first step. According to StatPearls, over 85 to 90 percent of patients with an acute herniated disc experience relief of symptoms within 6 to 12 weeks without treatment. Due to the high prevalence of disc herniation in routine neuroimaging of asymptomatic individuals, the recommendation is to avoid ordering imaging studies during this period as the study results will not alter management. In patients who are unresponsive to conservative treatment after two to three months, imaging is recommended.
Treatment: What Recovery Actually Looks Like
Most cases of sciatica respond to conservative treatment.
According to StatPearls (on lumbar disc herniation), primary care practitioners can begin treatment with a short course of rest if indicated, appropriate patient education, recommendations of physical exercises, and prescribing pain medications and physical therapy. In most cases, symptoms improve within a few weeks.
When conservative management is insufficient, additional options exist. According to Mayo Clinic Health System, some people find relief from targeted steroid injections into the lumbar spine directly around the nerve root that is irritated and compressed. Corticosteroid injections reduce pain by suppressing inflammation around the irritated nerve.
Surgery is reserved for specific situations. According to Mayo Clinic Health System, surgery is an excellent treatment for L5 or S1 radiculopathy when other conservative, nonsurgical treatments have not worked. There are generally two reasons for surgery. The first is significant weakness, such as a foot drop. In these cases, the sooner pressure is off the nerve, the more likely you can fully recover. The second, more common reason to have surgery is if conservative treatments have failed.
Recovery typically follows a pattern. The initial acute phase, with the most severe symptoms, may last a few weeks. The subacute phase, with gradually decreasing symptoms, may extend over one to three months. Full recovery may take longer if nerve compression was significant or if physical therapy is required to rebuild strength.
Why the First Few Weeks Matter
The window in which sciatica is most modifiable is the first several weeks after symptoms begin.
Conservative management works best when engaged early. Physical therapy, targeted exercises, and appropriate activity modification during the acute phase can prevent patterns of compensation that make recovery longer. Reassessment during this window allows for adjustments to treatment based on how the patient is responding.
Delayed intervention has different consequences. When acute nerve compression is left to persist, the risk of chronic pain patterns, persistent weakness, and prolonged recovery increases. Interventions that would have worked in the first month may be less effective in the fourth.
Consistent documentation across this window also matters clinically. Each visit produces a record of what symptoms are present, how they are progressing, and what treatment is being tried. Over weeks, this becomes the picture of what an injury actually required and how the patient responded.
